  • Close collaboration with the MDT to achieve bespoke goals
  • Collaboration with the education system, including primary and secondary schools, as well as mainstream and special needs schools
  • Support and advice for parents in their application for an Education and Health Care Plan (EHCP)
  • Engagement with the Local Authority and Early Help Practitioners for additional support
  • Advocating for the child, including attendance and provision of support regarding medical, educational and legal meetings
  • Assessing individual child needs and recruitment of suitable support workers
  • Provision of thorough induction, training, supervision and management of support workers/rehabilitation assistants
  • Ongoing coordination, monitoring and supervision of support packages
  • Developing bespoke Risk Assessments, Care Plans and monitoring procedures
  • Researching and procuring specialist equipment
  • Assisting with property purchase and adaptations to ensure accessibility
  • Support with family holidays
  • Extensive experience of preparation for and attendance at Safeguarding Meetings to ensure safety and well-being.
